Actamase is a combination injectable antibiotic containing Cefepime (a fourth-generation cephalosporin) and Tazobactam (a beta-lactamase inhibitor). It is used to treat severe bacterial infections caused by multidrug-resistant organisms.

Mechanism of Action

Cefepime:
-
A broad-spectrum cephalosporin that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs).

Effective against Gram-positive and Gram-negative bacteria, including Pseudomonas aeruginosa.

Tazobactam:

A beta-lactamase inhibitor that protects Cefepime from degradation by bacterial enzymes (e.g., ESBLs, AmpC beta-lactamases).

Extends the spectrum of Cefepime against resistant strains.

Dosage & Administration

Standard Adult Dose:
-
1g Cefepime + 0.125g Tazobactam or 2g Cefepime + 0.25g Tazobactam every 8–12 hours via IV infusion (over 30 mins).

Renal Impairment:

Dose adjustment required based on creatinine clearance (CrCl).

CrCl 30–60 mL/min: Reduce dose frequency to every 12–24 hrs.

CrCl < 30 mL/min: Further reduction needed.

Severe infections caused by beta-lactamase-producing bacteria, including:

Complicated intra-abdominal infections (cIAI)

Complicated urinary tract infections (cUTI)

Hospital-acquired pneumonia (HAP) & ventilator-associated pneumonia (VAP)

Storage

Unreconstituted vials: Store below 25°C, protect from moisture.

Reconstituted solution:

Stable for 24 hours at 2–8°C (refrigerated).

Use immediately if stored at room temperature.

Recommendations

Culture & sensitivity testing should guide therapy.

Monitor renal function (adjust dose in renal impairment).

Avoid prolonged use to prevent superinfections (e.g., C. difficile colitis).

Use with caution in patients with penicillin/cephalosporin allergy.
Important Note

Not effective against MRSAÂ (use additional coverage if MRSA suspected).

Cross-hypersensitivity may occur in penicillin-allergic patients.

Pregnancy & Lactation: Use only if clearly needed (Category B).
