Dosage & Administration

Standard Adult Dose:

  • 1g Imipenem + 1g Cilastatin IV every 6–8 hours (infused over 30–60 mins).

  • Max dose: 4g/day (severe infections).

Adjustments:

  • Renal impairment (CrCl <30 mL/min):

    • CrCl 15–30: 500 mg every 6–8 hrs.

    • CrCl <15: 250 mg every 6–12 hrs.

  • Hemodialysis: Administer after dialysis.

Pediatric Dose:

  • Children ≥3 months: 15–25 mg/kg every 6 hrs (max 4g/day).
