GERTIC INJ is a sterile, injectable solution containing Gentamicin Sulfate as the active ingredient. It is used for the treatment of severe bacterial infections caused by susceptible microorganisms.
Composition
-
Active Ingredient: Gentamicin Sulfate (equivalent to Gentamicin base) – 40 mg/mL or 80 mg/mL (as per strength).
-
Inactive Ingredients:Â Sodium metabisulfite (antioxidant), Water for Injection (WFI), and may contain small amounts of pH adjusters.
Mechanism of Action
-
Gentamicin is an aminoglycoside antibiotic that works by:
-
Binding to the 30S ribosomal subunit of bacteria.
-
Inhibiting protein synthesis, leading to bactericidal effects.
-
Effective against Gram-negative bacteria (e.g., E. coli, Pseudomonas, Proteus) and some Gram-positive organisms.
-
Dosage & Administration
-
Adults & Children:
-
IM (Intramuscular) or IV (Intravenous): 3–5 mg/kg/day in divided doses (every 8 hours).
-
Severe infections: Up to 5–7 mg/kg/day (monitor renal function).
-
-
Neonates: 4–5 mg/kg/day (adjusted based on weight and renal maturity).
-
Dosage must be adjusted in renal impairment.
Uses (Indications)
-
Severe bacterial infections (sepsis, pneumonia, meningitis).
-
Intra-abdominal infections (peritonitis).
-
Urinary tract infections (UTIs)Â caused by susceptible bacteria.
-
Skin & soft tissue infections, including burns/wound infections.
-
Post-surgical prophylaxis (in selected cases).
Storage
-
Store at controlled room temperature (20–25°C).
-
Protect from light and freezing.
-
Do not use if solution is cloudy or contains particles.
Recommendations
-
Administer under strict medical supervision.
-
Monitor renal function, hearing (ototoxicity), and serum levels (therapeutic range: 4–10 mcg/mL peak).
-
Avoid concurrent use with nephrotoxic drugs (e.g., vancomycin, diuretics).
Important Note
-
Contraindications:Â Hypersensitivity to aminoglycosides, myasthenia gravis.
-
Side Effects:Â Nephrotoxicity, ototoxicity (hearing loss), neuromuscular blockade, dizziness.
-
Pregnancy & Lactation: Use only if clearly needed (potential fetal harm).
-
Drug Interactions:Â Enhances neuromuscular blockade with anesthetics; avoid loop diuretics.
