Composition

  • Resistant to most beta-lactamases, but not metallo-beta-lactamases (MBLs).


Dosage & Administration

Standard Adult Dose:

  • 500 mg IV every 8 hours (for mild-moderate infections).

  • 1 g IV every 8 hours (for severe infections like meningitis, hospital-acquired pneumonia).

  • Infusion Time: Over 15–30 minutes (or extended infusion in critical cases).

Pediatric Dose:

  • 20 mg/kg every 8 hours (max 2 g/day for severe infections).

Renal Dose Adjustment:

  • CrCl 25–50 mL/min: 500 mg–1 g every 12 hours.

  • CrCl <25 mL/min: 250–500 mg every 24 hours.

Storage

  • Unreconstituted powder: Store below 25°C, protect from moisture.

  • Reconstituted solution:

    • Stable for 1 hour at RT (25°C) or 24 hours if refrigerated (2–8°C).

    • Do not freeze.


Recommendations

✔ Administer as IV infusion (15–30 min); extended infusion (3 hours) for critical infections. ✔ Monitor renal function, liver enzymes, and CBC during therapy. ✔ Not effective against MRSA (add vancomycin/linezolid if MRSA suspected).


Important Note

Contraindications:

  • Hypersensitivity to meropenem or other carbapenems (e.g., imipenem).

  • History of severe anaphylaxis to beta-lactams.

Side Effects:

  • Common: Diarrhea, nausea, headache, rash.
